Initial (>18years old) :
Opioid naive(use as the first opioid analgesic or use in patients who are not opioid tolerant): 10 mg q12h
Conversion from other oral oxycodone formulations to oxycodone CR:
Initiate oxycodone CR with 50% of the total daily oral oxycodone daily dose (mg/day) q12h.
Conversion from other opioids to oxycodone CR:
Discontinue all other around-the-clock opioids when oxycodone CR is initiated.
Initiate with 10 mg q12h.
Conversion from transdermal fentanyl patch to oxycodone CR: Conservative conversion factor :
oxycodone CR tablets 10 mg q12h= fentanyl 25 mcg/hour transdermal patch;
[Note]: Remove fentanyl patch at least 18 hours prior to starting oxycodone CR
Maintenance dose(>18years old):
Dosage adjustment (titration):
After initiation of oxycodone CR, adjust dose in increments (25% to 50%) no more frequently than every 1 to 2 days until desired pain control.
Dosage adjustment for concomitant therapy:
Concomitant CNS depressants: Initiate oxycodone CR with 33% to 50% of the calculated recommended dose. If reduced dose is less than smallest available dosage form consider alternative analgesic
Discontinuation of therapy:
Gradually titrate dose downward to prevent withdrawal signs/symptoms. Do not abruptly discontinue.

Nausea, vomiting, constipation, Drowsiness, headache, dizziness, Pruritus

嚴重呼吸抑制、已知或疑似麻痺性腸阻塞及腸胃道阻塞、意識受損或昏迷、循環性休克、週產期婦女、授乳婦女、急性或嚴重氣喘未接受監測或無急救設備狀態。

1.藥錠不得敲碎、溶解或咀嚼,否則將使oxycodone迅速釋出和吸收，使其濃度達致命程度。
2.發生oxycodone急性用藥過量時,將出現呼吸抑制,由嗜睡惡化為木僵或昏迷,骨骼肌無張力,皮膚濕冷,瞳孔收縮等症狀。
3.懷孕期間長期使用Oxycodone,可能會造成新生兒鴉片藥物戒斷症候群,若未即時發現與治療可能威脅生命。
4.可能引發嚴重低血壓(含姿態性低血壓及昏厥)。
5.可能會導致抽搐疾患症狀惡化及誘發癲癇發作或導致惡化。
6.使用本品時請避免同時使用Nalbuphine或Buprenorphine,否則將使止痛效果降低及/或引發戒斷症狀。 7.單次劑量超過40mg或單日總劑量超過80mg oxycodone僅可用於經確認可耐受該強度類鴉片類藥物的病患。可耐受類鴉片類藥物的病患係指：已在一週以上的時間接受每天60mg morphine/tab, 25mcg/hr Fentanyl patch或其他具有同等效果的類鴉片藥品。
