Gout
Adults:
initial 100mg qd increase at weekly intervals in increments of 100mg/day as needed to achieve desired serum uric acid level, max: 800mg/day
maintenance dose: 200 to 300 mg/day in mild gout; 400 to 600 mg/day in moderate to severe tophaceous gout.
Antineoplastic-induced hyperuricemia
Children <6yrs: 50mg tid, 6-12 yrs: 100mg tid or 300mg qd
Adults: 600-800mg/day, 12 hrs to 3 days prior to initiation of chemotherapy
抗尿酸結石
100-200mg qd

Rash, pruritus, renal failure, renal insufficiency, Stevens-Johnson syndrome, nausea, vomiting

1.如有皮疹或任何不良反應症狀(如發燒、肌痛)發生，須立即回診。
2.預防結石的產生，請每天喝充分的水份。
3.避免使用在無症狀之高尿酸血症
4.腎功能不良者須調整劑量。
5.HLA-B 5801基因檢測支付規定:
(1)適應症：新使用Allopurinol前且符合下列任一條件者：A.尿酸製造過多之痛風。B.經促尿酸排泄藥物使用最大耐受劑量後仍無法控制血液尿酸值6mg/dl以下，需使用Allopurinol者。C.具促尿酸排泄藥物使用禁忌症，如過敏、腎功能不全、肝功能不全等。D.具尿酸成分之尿路結石病史。E.接受化學治療之癌症病人用以預防急性尿酸引起腎病變。
(2)已連續使用Allopurinol一個月以上或曾使用Allopurinol累計滿三個月以上者，不予給付。
(3)為確保檢測品質，以LDT執行本項檢測，須具衛生福利部核准之實驗室開發檢測施行計畫。
(4)每人終生限給付一次，若檢測結果為陰性，且一個月內未開立Allopurinol者，不予給付；另檢測後一個月內未上傳檢測結果者，不予支付。
