Gestational Age (GA)≤34 weeks, Postnatal Age (PNA) ≤7 days: 25 mg/kg/dose q12h
GA≤34 weeks, PNA 8 to 28 days: 25 mg/kg/dose q8h
GA>34 weeks, PNA ≤7 days: 25 mg/kg/dose q8h
GA>34 weeks, PNA 8 to 28 days: 25 mg/kg/dose q6h

Pediatric(Infants, Children, and Adolescents)-General dosing:
Infants, Children, and Adolescents: IV, IM: 100 to 200 mg/kg/day in divided doses every 4 to 6 hours; maximum daily dose: 12 g/day

Adults: 1 to 2 g every 4 to 6 hours

Fever, rash, nausea, diarrhea, vomiting, eosinophilia, leukopenia, neutropenia, thrombocytopenia, agranulocytosis, hepatotoxicity, increased AST, hematuria, acute interstitial nephritis, hematuria, serum sickness-like reactions
