DVT prophylaxis
Hip replacement surgery:30 mg q12h or 40mg qd for at least 10 days or until risk of DVT has diminished or the patient is adequately anticoagulated on warfarin.
Knee replacement surgery: 30 mg q12h for at least 10 days or until risk of DVT has diminished or the patient is adequately anticoagulated on warfarin.
Abdominal surgery:40mg qd continue until risk of DVT has diminished (usually 7-10 days).
Treatment of DVT
1 mg/kg/dose q12h or 1.5mg/kg qd
Unstable angina, non-Q-wave MI
1mg/kg/dose q12h in conjunction with oral aspirin therapy (100-325 mg once daily); continue until clinical stabilization (a minimum of at least 2 days)

Hemorrhage, thrombocytopenia, fever, nausea

Active major bleeding, thrombocytopenia associated with positive antiplatelet antibody test in vitro induced by enoxaparin,severe renal dysfunction
